Thank you for your interest in referring client to My Signature Nutrition for nutrition assessment and counseling!  We appreciate and value your referral. We will provide the most current, evidence based nutritional care to your patients and keep you informed with their progress.  If you have any questions or concerns, please don’t hesitate to call us (903-312-8906) or email (info@mysignaturenutrition.com).

How to Refer a Patient:

Fax:  Please send us the client’s most recent progress note, pertinent labs, H&P, and other supporting documentation of diagnosis to 903-561-8799. Be sure to include the nutrition related ICD-10 diagnostic code related to the reason for referral. If you are unsure, you can always use the preventative code Z71.3 Dietary Surveillance and Counseling. For your convenience you can print our Medical Nutrition Therapy Referral Form, however this is not required. Once we receive your fax, we will contact the patient to schedule and notify you of the scheduled appointment.

Phone:  You can call us at 903-312-8906 and provide the necessary information over the phone or you can have your patient contact us directly.  For insurance reimbursement, it is helpful for our office to have your most recent progress notes and any related nutrition diagnosis.  This isn’t required, but may help with reimbursement.  Please fax them to us at 903-561-8799 or simply provide us with this information over the phone.

FAQs

Is a Registered Dietitian (RD) the same thing as a nutritionist?

No, A Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) is not the same thing as a nutritionist!  You could say that all dietitians are nutritionists, however not all nutritionists are dietitians. Registered Dietitian is a legal credential with a nationally recognized exam and standards that must be met to take the exam and maintain registration.  There are no specific requirements or recognized guidelines for credentials as a nutritionist.  A dietitian’s education provides academic and practical emphasis on the science of both food and nutrition.  This allows the dietitian to translate science-based messages that involve health needs, performance and wellness to more basic needs of what to eat, how to choose foods and ways to enhance health through lifestyle choices while separating food fact from food fad.  You can read more about what is a dietitian here.

Do you take insurance?

At this time we do not bill insurance, however we will provide your patient with a coded receipt to submit for insurance reimbursement.  Some patients receive up to 100% reimbursement for their appointment, while some patients solely have the payment applied towards their deductible.  Due to the wide variety in policies, reimbursement will depend on your patient’s specific policy.  If insurance is a factor in referring to us, please let us know!  We are considering joining insurance panels to help increase access to nutrition care in East Texas.

How much does it cost?

All new patients begin with a 90 minute nutrition assessment to review the patient’s health, weight, and nutritional status.

Follow up appointments are scheduled based on the amount of support your patient needs related to how much change they are trying to make, how quickly they are trying to change, and how much support they need to maintain this change.   Follow up appointments are typically 30, 45, or 60 minutes in length.  Frequency will also depend on the level of support they need and varies from weekly, biweekly, monthly, or simply as needed.

All appointments are billed based on a rate of $40 per 15 minute unit, or $160 an hour.  Amount will vary based on the length of the appointment.

What do you do in an appointment?

As a registered dietitian my goal is to help people create healthy food habits and feel confident in their eating choices.  To help a patient meet their nutrition goals we always start with a 90 minute nutrition assessment.  During this time we review their weight and diet history, health background, activity, and current nutritional intake.  We will then set long and short-term goals based on the kind of nutrition help they are looking for and provide education to help them meet their goals.  Since our treatment is highly individualized, the goals will depend on the client.  If you would like specific examples for your patient, please contact us directly at 903-312-8906.

What is a CEDRD?

A CEDRD is a certified eating disorders registered dietitian.  It is a credential obtained through the International Association of Eating Disorders Professionals that recognizes excellence in the treatment of eating disorders.   You can read more about the CEDRD credential in the resource below titled: The CEDRD in Eating Disorder Care.
